Joining Form

The below form must be completed as part of the full joining process and forms our contract.

All details are of the proposed member so a parent or guardian must input for under 16s (if you are a family, please submit one form for each of you).
Joining fees are per person and payable by direct debit upon joining.

I choose to join Bushido Hombu Dojo as a committed student of karate-do.

I understand that training here carries expectations. By signing this declaration, I agree to:

– Train with respect for myself, my instructors, my training partners, and the dojo.
– Uphold the values of Bushido: integrity, courage, compassion, respect, honesty, honour, and loyalty.
– Show discipline and effort, turn up on time, train with focus, and maintain a positive attitude.
– Follow dojo etiquette and safety rules to protect myself and others at all times.
– Represent the club with dignity inside and outside the dojo.
– Commit to personal growth physically, mentally, and morally.

I understand that karate-do is a journey that requires consistency. I accept responsibility for my own development and conduct while training at Bushido Hombu Dojo.
