Trial sessions are by appointment only, so it is important that the below form is completed and payment made before attending a trial session. Please allow 5 working days between your application and the date you wish to attend, the club is run by volunteers therefore emails are not constantly monitored.

All details are of the proposed member so a parent or guardian must input for under 16s (if you are a family, please submit one form for each of you).The disclosure of medical issues is of paramount importance to ensure we are able to keep you and other members safe.
