Trial sessions are by appointment only, so it is important that the below form is completed and payment made before attending a trial session.

All details are of the proposed member so a parent or guardian must input for under 16s (if you are a family, please submit one form for each of you).The disclosure of medical issues is of paramount importance to ensure we are able to keep you and other members safe.

We ask for payment upfront so we can guarantee your place and have everything ready for you on the night. It takes 2 minutes and means we’re both committed. Only one payment required per family.
